All candidates for middle school and high school athletic teams (grades 6–12) during the 2026-2027 school year must complete a physical examination performed by a physician (MD or DO) holding an unlimited license to practice medicine, nurse practitioner or physician assistant. The examination must take place—and the provider’s signature, with license number, must be dated after April 1, 2026. Sports physical forms may be available through your school’s athletic department. In order for a student-athlete to be eligible for any sport, a completed sports physical form must be returned to the appropriate athletic office prior to the first practice. Riverview Health will provide physical examinations for prospective student athletes at a cost of $30.Once you complete the registration form, you will receive a receipt via email. Please print this receipt and bring it with you to the screening as proof-of-payment. Visa and MasterCard are accepted. Online registration will close at 5 p.m. on Thursday, April 30.
Please print a copy of the physical form and have your athlete bring the copy with them on May 1. The physical form must have a completed medical history section and a parent’s signature.

Dr. Trenton Schmale is a family medicine physician at Sheridan Family Medicine. He is passionate about sports medicine and played football at Carmel High School and atTaylor University. He is board-certified in family medicine and completed a fellowship at University of Missouri Kansas City where he specialized in sports medicine, preventive medicine, outpatient procedures and ultrasound. Dr. Schmale attended medical school at Marian University College of Osteopathic Medicine.
