Patient Referrals

Patients may be referred to the program in a hospital, in a skilled nursing facility, or home. Patients currently at home do not need a 3-day hospital stay to be admitted to our unit. Complimentary assessments can be done in the home. Please call 317-776-7901 to schedule a patient referral.
